Register Online with Cornbrook Medical Practice

We welcome new patients!

PLEASE NOTE: We are experiencing very high numbers of requests at the moment. These are currently taking around three weeks to process. Please allow 21 days from submitting your request before contacting us.

We also receive many requests with missing information, resulting in processing delays or rejections. Please read each section carefully & complete the form as fully as possible.

 

We offer a GP service to residents of Hulme and Manchester City Centre, those with an M15 postcode.

You can easily register with us online by completing the form below. Once you complete the form, you will be registered within 7 days. You will receive a welcome text informing you of your named GP and an email with log in details for Patient Access. 

If you live outside of the M15 postcode but still want to register with us, you may register as an out of area patient. We have many patients who work in the City Centre and chose to register with us for convenience. If you are deemed out of area, you will receive a text message informing you that you will not be eligible for home visits from the team. 

Please download and complete the following word document. Once you have completed this, you can send it to mhcc.registrations.cornbrookmedicalpractice@nhs.net along with 1 form of ID and 1 form of proof of address. ID can be:

1. Passport

2. Driving Licence

3. Birth Certificate

4. National ID Document

Proof of address can be:

1. Driving Licence

2. Utility Bill dated in last 3 months such a council tax, gas or electricity

3. Bank Statement dated within the last 12 months confirming address

4. Tenancy agreement

Once you email these forms it will take up to a month to register you on our system. You will receive a welcome email with your named GP, a text message if you are out of area. If you do not receive these within a month of registering, please contact the practice by telephone.
