See local and national support for carers
We want to make sure our carer's register is up to date so if you are a carer or have a carer please let reception know to update your records. We can also help signpost you for support with finances and support around the house.
Did you know that as a carer you can get an annual health check for free along with a flu vaccination with our Health Care Assistants? Simply contact reception to book this in for you.
We can also help you access the extended access GP appointments so you can access GP and nurse services when you have some time to yourself. Speak to reception for more infomation.

We offer coil and implant fittings to all residents of Manchester.

Flu Vaccination
These will be arranged in the autumn and winter and more information will be published seasonally. Eligble patients will be invited automatically. If you have not received an invitation and think that you should have please contact the practice.

Gender Identity Clinic
We are one of the founding members of Indigo Gender Identity Service.
The service has a base at Cornbrook Medical Practice. One of our GPs, Dr Philippa James, is a specialist in Trans Health.
Bridging Prescription Service Update
We would like to inform our patients that the practice is no longer providing new hormone bridging prescriptions.
Patients who are already on the existing waiting list will still be seen, but we are unable to add any new patients. This decision has been made in light of reduced waiting times for Indigo Gender Services and the upcoming retirement of Dr James later this year.
We remain committed to supporting our transgender and gender-diverse patients. Although we are not initiating new bridging prescriptions, we continue to offer:
- Support and advice around gender-affirming care
- General health care for all patients
- Referrals to Indigo Gender Services, if requested

Homeless Access Hub
We support those facing homelessness, asylum seekers, refugees and we offer flexible registration for all.
We run a weekly drop-in clinic at Cornerstone Day Centre/Morning Star Hostel every Tuesday at 12:30pm. During winter we offer Flu Vaccines and where possible COVID-19 vaccines.

We offer free HPV vaccines for those who are eligible.
Long Term Condition Management
We manage your long-term conditions at the practice as a practice team. You may see our Pharmacists, Nurses, Health Care Assistants, GPs or any other team member best placed to help you manage your long-term condition.
A long-term condition is a physical problem that requires ongoing management for life, this includes Asthma, Mental Health, Dementia, COPD, Diabetes, Hypertension.

National Databank
Free SIM Cards & Mobile Data at Cornbrook Medical Practice
Cornbrook Medical Practice is now an approved National Databank hub, offering free SIM cards and mobile data to patients who are struggling to afford internet access.
The National Databank, created by Good Things Foundation, provides free mobile data, texts and calls to people experiencing data poverty — working “like a foodbank but for mobile data.”
Major networks including Virgin Media O2, Vodafone and Three donate SIM cards and data packages, which are distributed through community hubs like ours to help people stay connected.
Who can receive support?
You may be eligible if you:
- Cannot afford mobile data or a phone contract
- Have limited or no internet access at home
- Need help staying connected to essential services
(These criteria are based on national guidance for access to Databank SIM cards.)

Just come to reception and ask about the National Databank SIM cards.
Our team will check eligibility and provide support in a friendly, confidential way.

NHS Health Checks
A free health check for patients aged over 40. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.
Pregnancy and Baby Clinic
There's a lot to remember when you become pregnant or give birth. The NHS website has guides to help you manage the health of you and your baby and explain the baby clinic immunisation process.

We offer sexual health screening and treatment. You don't have to be registered with the practice, just a resident in Manchester.

One of our sites is in the heart of University Green. We welcome and register all students. We are at freshers fair every year talking to you about our services.

ADHD and Autism Referrals
At Cornbrook Medical Practice, we recognise that seeking an assessment for ADHD or Autism can feel overwhelming, emotional, or even daunting. Many patients come to us after years of struggling, masking, or feeling unheard. We want you to know that we take neurodiversity seriously and aim to support you with care, respect, and understanding.
While we do not diagnose ADHD or Autism within the practice, we are committed to helping you access appropriate assessment services and guiding you through your referral options.
Understanding the Referral Process
ADHD and Autism assessments require detailed information about how symptoms affect you, both now and earlier in life. This helps specialist services make safe and accurate decisions.
We understand that:
- Remembering childhood experiences can be difficult
- Putting things into words isn’t always easy
- Forms can feel intimidating or exhausting
If you are struggling with the process, please let us know — we will do our best to support you within the limits of NHS services.
Your Referral Options
You have three choices when being referred for ADHD or Autism assessment:
1. NHS Referral
- This is a referral to local NHS assessment services.
- Waiting times are currently very long.
- You will be asked to complete a questionnaire or referral form.
- It is important to be as specific and detailed as possible, including examples of how symptoms affect daily life, work, education, and relationships. You will also need to provide examples relating to growing up.
- Once completed, we will send the referral on your behalf.
Please note: referrals with limited detail may be delayed or declined by assessment services.
2. Right to Choose (RTC)
Under the NHS Right to Choose scheme, patients can choose from several private providers who hold NHS contracts, meaning the assessment is free of charge.
- These providers are independent companies.
- Because of this, we are unable to recommend or advise on which provider to choose.
- Patients are responsible for selecting their preferred service.
- You will need to discuss payment for medication with the provider
When choosing a provider, you may want to consider:
- Current waiting times
- Whether they assess ADHD, Autism, or both
- What follow-up support they offer after diagnosis
Once you have made your choice, please contact the surgery and we will be happy to refer you.
3. Private Referral
- You may choose to pursue a fully private assessment.
- We can provide a referral letter if requested.
- There may be a charge for this service.
- You will need to provide details of the service you wish to be referred to.
Medication and Shared Care
Please be aware that Cornbrook Medical Practice is only able to accept Shared Care Protocol from NHS-commissioned services for ADHD or Autism medication.
This means:
- We cannot take over prescribing from private or Right to Choose providers
- Ongoing prescribing remains the responsibility of the diagnosing service
- You will need to discuss the possible cost of medication with the Right to Choose provider
We understand this can be disappointing, and we encourage patients to consider this carefully when choosing a provider.

We believe ADHD and Autism are part of natural human diversity. Our team aims to:
- Treat all patients with dignity and respect
- Listen without judgement
- Acknowledge the challenges neurodivergent patients may face
- Support reasonable adjustments where possible

Once you have made your choice please contact the practice and let us know which pathway you would like to use.
- If you select the NHS pathway, we will provide you with the relevant form and on return send the referral off for you.
- If you select the Right to Choose pathway you will need to let us know which service you would like to be referred to and enclose any documents required.
- If you select the Private pathway please let us know the necessary paperwork for us to complete.
